- Nursing Home or Hospital experience. Intership is sufficient. (Required)
- BSW or MSW in related field. (Required)
- Bi-lingual English/Spanish (preferred but not required)
- Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
- Initiating appropriate interventions for residents experiencing mental or behavioral health issues
- Identifying individual social and emotional needs
- Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility
- Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
- Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
Salary : $60,000 - $65,000
