Responsibilities
- Develop and implement individualized care plans tailored to each person's needs.
- Provide direct support to clients, assisting with daily activities and promoting independence.
- Monitor and document client progress, adjusting care strategies as necessary.
- Collaborate with team members, families, and community resources to ensure comprehensive support.
- Ensure compliance with all relevant regulations including HIPAA and safety standards.
- Advocate for clients’ rights and promote their dignity in all interactions.
- Assist in training staff and new team members on best practices.
- Maintain accurate records using EMR systems and other documentation tools.
- Qualifications
- Bachelor’s degree in a related field (Social work, Psychology, Human services, etc) preferred not required
- Prior experience in residential program management or related field
- knowledge of applicable regulation and requirements for residential programs
- Strong organizational and time management skills.
- Excellent communication and interpersonal skills to effectively work with individuals, families and staff
- Ability to prioritize task and work independently as well as part of a team
- Proficiency in computer software application such as Microsoft Word
- Valid driver’s licenses and reliable transportation
