Duties
- Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to identify treatment needs.
- Develop and implement tailored care plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and patient education.
- Manage patient rehabilitation programs in our skilled nursing facility.
- Document clinical findings accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
- Coordinate discharge planning and collaborate with healthcare professionals to ensure seamless patient transitions between care settings.
- Conduct treatment sessions focused on geriatrics, and other specialized areas to optimize recovery outcomes.
- Engage in ongoing patient communication to motivate adherence to therapy regimens and address concerns effectively.
Qualifications
- Valid state licensure as a licensed Physical Therapist (PT) along with the NPTE Exam.
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
- Demonstrated ability to develop effective treatment plans tailored to individual patient needs.
- Experience working with geriatric patients, pediatric populations, or neurological conditions is highly desirable.
- Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and care coordination.
- Excellent client communication skills coupled with the ability to collaborate effectively with healthcare teams.
- Knowledge of discharge planning processes and outpatient rehabilitation center protocols.
