The Nurse for the Pharmacologic Management/MED SOM Program participates with psychiatrists in providing and managing the mental health care needs of Ravenwood Health clients. The Nurse provides nursing services including medication teaching and general health monitoring/screening and helps with coordination of care with other health care providers. Under the supervision of the psychiatrist, the nurse assists with the assessment and ongoing medication management for those clients receiving psychiatric care. The nurse manages the Central Pharmacy medications and provides organizational support to the program.

JOB DESCRIPTION

Key Responsibilities
- Maintains current clinical competence by participation in appropriate training. In addition, provides the agency of proof of participation in such activities by providing compilation of continuing education credits.
- Maintains an acceptable productivity level (billable services).
- Accepts and documents verbal orders from Ravenwood Psychiatrists.
- Manages all business related to Central Pharmacy.
- Assists the psychiatrists in direct care of patients via providing pre-assessment interviews that is appropriate in connection with nursing activities (under the supervision of Dr. Rodio) and management of Pharm/Management cases.
- Administer medication under the direction of the psychiatrist in accordance with Chapter 4723 of the Ohio Revised Code.
- Adheres to agency Policy and Procedure, and HIPAA mandates.
- Participates in the development of the patient’s ISP for those Pharm/Management patient’s only.
- Provides supplemental medication education to patients when indicated.
- Displays professionalism and respect in interactions with patients and staff.
- Reviews Health History Questionnaire forms including nurse recommendations with client and provides additional health referral information as needed.
- Completes and records Second Generation Antipsychotic measurements on those client’s indicated on a regular basis.
- Performs wellness checks/ health screening’s including Vital Signs, Weight Check, Pain level and records information in clinical record
- Provides direct patient care, evaluates outcomes and adjusts nursing care as indicated to ensure optimal patient care.
- Other duties as required.
- Must have excellent written and verbal communication skills
- Demonstrate knowledge and remain current on information pertaining to mental health treatment.
- Ability to prioritize effectively and work independently.
- Is effective in assessing and responding to client’s needs and problems.
- Regularly attends professional staff meetings, coordinating treatment and leadership in order to facilitate the ongoing care psychiatric care of the patient.
- Demonstrate strong communication skills by returning and responding to phone calls or emails within 24 hours and other correspondence in a timely manner.
- Demonstrates good decision making and problem solving skills.
- Maintains clinical record as required.
- Is dependable in attendance.
- Demonstrates initiative and contributes to ideas and suggestions to improve program operations and quality improvement.
Required Education And Experience
- Current and valid Ohio Nursing License
- Graduate of an accredited School of Nursing
- Mental health Experience is preferred
- Drug and Alcohol knowledge/experience is a plus
- Ancillary training/certifications: CPR, First-Aid, Non-Violent Crisis Intervention
- Maintain ongoing continued education as required
- All employees who provide mental health services shall be qualified to provide those services according to Chapter 5122-23 of the administrative code.
- Clinical services can be only provided by staff members who have their license verified and have been approved by the Competency Assessment Committee and/or are privileged by the Board of Directors, or who are working under the supervision of a staff member whose competency has been approved and/or they are a privileged staff member.
- Ability to work in a past paced environment; meeting with clients as scheduled
- Required to communicate with clients and other staff members
- Travel to other Ravenwood offices is necessary at times
- Work hours may vary based on client needs with or without prior notice
