Raleigh Medical Group, PA is seeking an experienced and strategic Vice President of Value Based Care (VBC) to provide executive leadership for our value-based care initiatives across the organization. This senior leadership role is responsible for advancing high-quality, cost-conscious care delivery through population health, care management, quality improvement programs, and incentive-based reimbursement models.
Working collaboratively with clinical leadership, operational teams, Accountable Care Organizations (ACOs), and payer partners, the Vice President of Value Based Care will develop, implement, and optimize innovative care strategies that improve outcomes, drive revenue opportunities, and enhance the health of the communities we serve.
Responsibilities
- Lead strategic and operational planning for value-based care initiatives, ensuring alignment with quality performance goals and regulatory requirements.
- Drive engagement with Accountable Care Organizations (ACOs), including collaboration on initiatives, monitoring performance, and ensuring adherence to quality standards.
- Provide executive oversight of population health, care management, chronic condition management, quality improvement, and incentive-based programs
- Partner with Clinical Directors, Population Health Specialists, ACO representatives, and payer organizations to advance organizational goals and performance
- Monitor and analyze quality, utilization, and incentive program outcomes across Medicare, commercial, employer-sponsored, and other populations
- Collaborate with Health Information Technology and EHR leadership to optimize data systems, reporting capabilities, and performance tracking
- Provide leadership, coaching, and education to clinicians and operational teams regarding value-based care strategies and quality initiatives
- Identify and develop opportunities for revenue growth through care management services, shared savings arrangements, and quality incentive programs
- Oversee Chronic Condition Management (CCM) programs and support innovative care model development to improve long-term patient outcomes.
- Ensure compliance with all applicable regulatory requirements, ACO standards, and healthcare industry best practices
- Support quality and cost-control initiatives related to Raleigh Medical Group's self-funded employee health plan.
- Foster strong partnerships across clinical, operational, and external stakeholder groups.
Qualifications
Required:
- Master of Public Health degree
- 10 years of progressive experience in healthcare quality improvement, population health, care management, value-based care, or incentive-based reimbursement programs.
- Minimum of 5 years administering Population Health, Quality, Care Management, and Incentive Programs
- 10 years of leadership and management experience
- Experience working with Accountable Care Organizations (ACOs), quality programs, and regulatory frameworks
- Strong strategic planning, analytical, problem-solving, and organizational skills
- Exceptional communication, relationship-building, and leadership abilities
Preferred:
- Experience with Athena EHR
- Experience working for a payer or health plan within value-based care programs.
- Experience designing, implementing, and leading Transition of Care (TOC) and Chronic Condition Management (CCM) programs.
- Experience within a physician-led independent medical practice environment.
