Job Summary
We are seeking a Licensed Clinical Social Worker (LCSW) to join our expanding team. This role is especially critical in providing time-sensitive follow-up hospital visits, delivered via telehealth or field-based services, to ensure continuity of care and reduce risk of readmission. In addition, the clinician will provide therapeutic services, either through telehealth or in-person field-based sessions, based on the clinician’s comfort level and the needs of the client to support stabilization, recovery, and long-term wellness.
Responsibilities
- Provide individual counseling and therapeutic support to clients across diverse settings, including hospitals, housing programs, and community sites.
- Conduct timely hospital follow-up visits to ensure smooth care transitions and address urgent behavioral health needs.
- Perform comprehensive psychosocial assessments, treatment planning, and progress reviews.
- Collaborate with case managers, medical staff, and community partners to support wraparound care.
- Maintain accurate and up-to-date clinical documentation in Rainbow Health’s electronic health record (EHR).
- Uphold the highest ethical and professional standards, including compliance with HIPAA and organizational policies.
- Maintain updated CAQH profile, licensure requirements, NPI, and professional credentials.
Requirements
- Active LCSW license in the State of Hawaii.
- Must be physically located on the Big Island of Hawaii to provide in-person and field-based services as needed.
- Experience in crisis response, hospital-based social work, or integrated behavioral health strongly preferred.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Must provide and maintain malpractice/professional liability insurance.
- Commitment to providing evidence-based, trauma-informed, and culturally responsive care.
- Proficiency with technology platforms (e.g., Gmail, TherapyNotes, Microsoft/Google Workspace).
- Flexibility to respond to time-sensitive referrals and perform telehealth or field-based follow-up visits.
