We serve some of the most geographically isolated and high-need communities in the nation, with approximately 65% of our clients identifying as Native Hawaiian or other Pacific Islander. Our programs are designed to meet people where they are in shelters, transitional housing, hospitals, and the community with compassion, dignity, and trauma-informed care.

Job Summary

We are seeking a Licensed Clinical Social Worker (LCSW) to join our expanding team. This role is especially critical in providing time-sensitive follow-up hospital visits, delivered via telehealth or field-based services, to ensure continuity of care and reduce risk of readmission. In addition, the clinician will provide therapeutic services, either through telehealth or in-person field-based sessions, based on the clinician’s comfort level and the needs of the client to support stabilization, recovery, and long-term wellness.

Responsibilities

• Provide individual counseling and therapeutic support to clients across diverse settings, including hospitals, housing programs, and community sites.
• Conduct timely hospital follow-up visits to ensure smooth care transitions and address urgent behavioral health needs.
• Perform comprehensive psychosocial assessments, treatment planning, and progress reviews.
• Collaborate with case managers, medical staff, and community partners to support wraparound care.
• Maintain accurate and up-to-date clinical documentation in Rainbow Health’s electronic health record (EHR).
• Uphold the highest ethical and professional standards, including compliance with HIPAA and organizational policies.
• Maintain updated CAQH profile, licensure requirements, NPI, and professional credentials.

Requirements

• Active LCSW license in the State of Hawaii.
• Must be physically located on the Big Island of Hawaii to provide in-person and field-based services as needed.
• Experience in crisis response, hospital-based social work, or integrated behavioral health strongly preferred.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Must provide and maintain malpractice/professional liability insurance.
• Commitment to providing evidence-based, trauma-informed, and culturally responsive care.
• Proficiency with technology platforms (e.g., Gmail, TherapyNotes, Microsoft/Google Workspace).
• Flexibility to respond to time-sensitive referrals and perform telehealth or field-based follow-up visits.
