What are the responsibilities and job description for the CARE COORDINATOR position at QUINCO MENTAL HEALTH CENTER?
Summary: This position manages an assigned caseload of TennCare and Behavioral Health Safety Net clients with the highest behavioral health needs by coordinating their behavioral and physical health services.
Essential Duties and Responsibilities include the following:
• Complete a psychosocial assessment with each client referred to the Care Coordination Program to aid in identifying the client’s strengths, goals, and needs.
• Participate at least once weekly in regular interdisciplinary staff meetings at your base location.
• Accept newly assigned cases in which a master or licensed clinician has completed a diagnostic assessment.
• Develop individualized, measurable, and objective goals and plans to meet the client’s identified needs with the client’s input.
• Provide ongoing support and care coordination per the problems, needs, and strategies identified within the care plan to help clients achieve their goals and objectives.
• Document all service contacts on time, including face-to-face interviews, collateral and networking contacts, and correspondence per agency policies and procedures.
• Meet regularly with the immediate supervisor to enhance professional growth, review, and process the provision of care coordination services, and deal with relevant administrative issues.
• Maintain close relationship with the local medication clinic staff for input regarding medication compliance, side effects of medication(s), and medication changes and alert these members of the staff of any changes in the client’s adjustment which might suggest decompensation and a need for reassessment by the medical staff.
• Complete reports, care plans, progress notes, and other documents professionally and according to agency policy and procedures.
• Utilize their vehicle to provide services to clients in the community.
• Be available for crisis management after regular work hours.
• Meet with each assigned client a minimum of one time per month, in person or by phone, but must provide at least one face-to-face service every 90 days.
• Perform other duties as assigned by the supervisor, which is consistent with the position and in compliance with agency policies and procedures.
Supervisory Responsibilities: None.
Qualifications:Qualifications: Must possess a combination of mental health work experience, relevant education, and training which indicates the knowledge, skills, and abilities necessary to perform essential job duties. Bachelor’s degree or higher in a health-related field of counseling, psychology, social work, sociology, or a behavioral science field or must be licensed as a Registered Nurse.
Individuals with an alternate bachelor’s degree or LPN must have fifteen (15) semester hours of coursework in behavioral health, and at least one (1) year of work experience in the behavioral health setting may be considered.
Essential Knowledge & Abilities: Working knowledge of the nature of mental illness and related treatment modalities, interventions, and techniques; different types of assessments and their uses in treatment planning; consumers’ rights; local community resources and service delivery systems such as housing, social, welfare, educational, etc.; client record documentation requirements; and client care plan development and implementation.
Abilities: Demonstrate ability to interview and assess clients, use appropriate assessment tools, and observe, record, and report on an individual’s functioning; to read and understand assessments, evaluations, observation, and use in developing a client’s care plan; to identify community resources and services for clients and coordinate the provision of services; to establish effective working relationships with internal agency staff as well as with relevant community organizations; interact positively with consumers and their families, work as a team member, communicate effectively, verbally and in writing, to maintain confidentiality, and to work independently under general supervision.
