Registered Nurse (RN) Case Manager

The Registered Nurse (RN) Case Manager plays a key role in coordinating and managing patient care throughout the healthcare continuum. Working in an acute care hospital environment, this position focuses on care coordination, utilization review, discharge planning, and ensuring patients receive appropriate services to achieve optimal outcomes.

Assignment Details

  • Contract Length: 13 Weeks
  • Shift: Day 5x8-Hour (08:00 - 16:30)
  • Setting: Acute Care Hospital

 

Key Responsibilities

  • Coordinate and oversee patient care plans throughout hospitalization
  • Perform comprehensive case management and utilization review activities
  • Facilitate discharge planning and coordinate post-acute care services
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals
  • Assess patient needs and identify barriers to care and discharge
  • Ensure appropriate utilization of healthcare resources and services
  • Maintain accurate and timely documentation in the electronic medical record (EMR/EHR)
  • Monitor patient progress and adjust care plans as needed
  • Advocate for patients to ensure safe and effective transitions of care
  • Support quality improvement initiatives and patient outcome goals

Requirements

  • Active Registered Nurse (RN) License
  • Minimum 2 years of Acute Care Hospital Case Management experience
  • Experience with Care Coordination, Utilization Review, and Discharge Planning
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and organizational abilities
  • Ability to work effectively in a fast-paced acute care environment

Preferred Qualifications

  • Experience coordinating interdisciplinary care teams
  • Familiarity with hospital case management workflows and healthcare regulations
  • Proficiency with Electronic Health Records (EHR/EMR) systems
  • Strong patient advocacy and problem-solving skills
