About us
Queens Centers for Progress, an Agency providing person centered services for adults and children with developmental and intellectual disabilities is seeking a compassionate and dedicated skilled Licensed Social Worker for our Article 16 Clinic fully in-person.
SUMMARY:
Under direct supervision, the Licensed Social Worker will be responsible for conducting clinical evaluations and delivering structured interventions to individuals with Intellectual and Developmental disabilities (IDD), as well as, their families and/or significant other. Services are provided primarily through individual and family counseling with the goal of resolving or alleviating personal, emotional or social challenges. Services are provided on site in our Article 16 Clinic.
DUTIES:
- Provide information/referral service to persons with IDD and their families
- Conduct intake/screening to potential clients and assist in defining the presenting problem
- Conduct psycho-social evaluations to determine OPWDD eligibility and recommend services needed by client
- Provide individual and/or family counseling services to those individuals
- Develop and review treatment plans based on a thorough need assessment
- Update and maintain case records in accordance with Part 679 regulations.
QUAIFICATIONS:
To be successful in this role, you should have the following qualifications:
- Master’s Degree in Social Work(MSW) from an accredited institution
- One year work experience in IDD field
- Current NYS licensure (LMSW or LCSW) required
- Excellent communication and interpersonal skills.
Preferred skills: Bi-lingual (English/Spanish)
