SUMMARY OF POSITION:
We are looking for a dedicated and knowledgeable Nutritionist to join our team. In this essential role, you will conduct comprehensive nutrition assessments, provide counseling, and offer ongoing support to individuals receiving clinic services. The focus is on promoting overall health, preventing nutrition-related complications, and ensuring dietary needs are met while considering each individual’s medical, behavioral, and developmental needs.
Schedule: 5-7 hours a week
KEY RESPONSIBILITIES:
- Conduct detailed nutrition assessments incorporating medical history and dietary intake, and develop individualized nutrition care plans
- Maintain accurate and timely documentation, including progress notes, care plans, and evaluation reports, in compliance with Article 16 Clinic regulations (NYS OPWDD), Medicaid/insurance requirements, and clinic policies
- Promote healthy eating habits tailored to each individual’s developmental level and cultural background
- Provide both individual and group nutrition counseling using a person-centered approach, adjusting plans as needed to support positive health outcomes
- Develop and deliver nutrition education programs, workshops, and materials for individuals, families, and staff
QUALIFICATIONS:
- Bachelor’s or Master’s degree in Nutrition, Dietetics, or a related field
- Credentialed as a Registered Dietitian Nutritionist (RDN) or Certified Dietitian/Nutritionist (CDN) in New York
- Experience with individuals with intellectual and developmental disabilities (IDD)
- Excellent communication skills for health coaching and interdisciplinary collaboration
