Job Title: Licensed Clinical Social Worker (LCSW)
Contract Length: 13 Weeks
Schedule: Monday–Friday, 12:00 PM – 8:30 PM
Responsibilities:
- Conduct psychosocial assessments and develop individualized care plans.
- Provide counseling, crisis intervention, and emotional support to patients and families.
- Coordinate discharge planning and connect patients with appropriate community resources.
- Collaborate with physicians, nurses, case managers, and interdisciplinary care teams.
- Assist patients with navigating healthcare services and addressing social determinants of health.
Required Qualifications:
- Current Georgia Licensed Clinical Social Worker (LCSW) license.
- Current AHA Basic Life Support (BLS) certification.
- Minimum 2 years of recent Social Work experience within the specialty.
Recent acute care hospital experience required.
