What are the responsibilities and job description for the PHARMACIST position at PULASKI MEMORIAL HOSPITAL?
The Outpatient Pharmacist is part of the client-based on-site operations team which is the frontline responsible for delivery of specialty pharmacy services as part of an integrated care team. The Clinical Pharmacist owns the clinical outcomes for each patient that we serve. The Clinical Pharmacist assists with non-clinical tasks that their patients require, including prior authorizations or dispensing activities to ensue the best clinical outcomes. This position will also assume responsibility for developing and implementing evidence-based clinical protocols, as well as driving clinical quality improvement at the customer site. Ability to work with other members of CPS and hospital staff.
B.S. in Pharmacy from an accredited college of pharmacy. Advanced degree (e.g., PharmD, M.S.) preferred. Licensed Pharmacist in the state of the site in which the Pharmacist is supporting. Two years related pharmacy experience is preferred.
