- Serve as the primary clinical partner, delivering personalized in-home care to promote recovery, safety, and independence.
- Conduct comprehensive assessments to develop patient-centered care plans addressing physical, emotional, and environmental needs.
- Perform skilled interventions such as wound care, infusions, medication management, and chronic disease management.
- Educate and empower patients and caregivers, ensuring confident management of symptoms, medications, and daily routines.
- Monitor patient progress, identify early changes, and communicate with care teams to adjust plans and prevent hospitalizations.
- Coordinate care across providers, ensuring continuity and high-quality outcomes.
- Complete accurate documentation supporting quality, compliance, and measurable clinical results.
- Active RN license in the practice state.
- At least one year of clinical nursing experience.
- Patient-focused, organized, and motivated to deliver high-quality care.
- Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain).
