- Assist with transfers, repositioning, and ambulation to support patient safety and comfort.
- Prepare meals considering client preferences and dietary needs.
- Offer companionship and engage clients in activities tailored to their interests.
- Perform light housekeeping duties and maintain accurate records of care provided.
- Monitor and report changes in clients' physical or psychosocial conditions.
- Follow the care plan strictly, including documenting tasks via EVV app or time sheets, and uphold patient confidentiality and privacy.
- Ensure client independence within their abilities and respond to any concerns or safety issues promptly.
- Current certification with DFS.
- High school diploma or GED preferred.
- Effective communication skills, including reading, writing, and following instructions.
- Ability to perform physical tasks such as lifting, bending, and standing.
- Sympathetic attitude towards patient care and safety.
