- Managing individualized patient care plans to promote safety and independence at home.
- Providing skilled nursing services such as medication administration, wound care, and patient education.
- Conducting assessments, monitoring patient progress, and adjusting care plans accordingly.
- Acting as the primary liaison between patients, families, physicians, and the care team.
- Coordinating care with interdisciplinary team members and documenting clinical activities accurately.
- Educating patients and families on health management and safety procedures.
- Communicating patient status updates to ensure timely interventions.
- Active RN license in Michigan.
- Over one year of high acuity nursing experience.
- Compassionate, flexible, patient-centered approach.
- Strong organizational, communication, and clinical decision-making skills.
- Valid driver’s license and current CPR certification.
