Role: Provide occupational therapy services in patients' homes, assessing functional status, activities of daily living, fine motor skills, and home safety. Collaborate with healthcare team to develop and implement personalized care plans, monitor patient progress, and document responses to treatment. Conduct evaluations and recommend adaptive equipment as needed.

Responsibilities
- Perform assessments and evaluations within the scope of practice and according to state laws
- Develop and modify therapy plans in consultation with physicians
- Observe, record, and communicate patient responses and condition changes
- Apply diagnostic and prognostic tests to evaluate functional ability
- Current Occupational Therapy license in the practice state
- CPR certification
- Valid driver’s license, vehicle insurance, or access to reliable transportation
- Ability to work effectively in diverse home environments
