Responsibilities
- Conduct comprehensive in-home assessments, including medical history, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based screenings and point-of-care testing as needed.
- Manage patients through acute episodes, chronic condition exacerbations, and short-term rehabilitation.
- Ensure accurate documentation of clinical encounters, including ICD-10 coding.
- Collaborate with interdisciplinary care teams and conduct advanced care planning conversations.
- Provide counseling and education to patients and caregivers.
- Active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care for NPs; NCCPA certification and state licensure for PAs.
- Unrestricted licensure and DEA registration (or ability to obtain).
- Reliable transportation and active driver's license.
- Ability to navigate varied environments and perform physical duties as needed.
