Role Overview: The Nurse Navigator is a registered nurse with oncology expertise responsible for coordinating and guiding patients through the cancer care continuum.

- Coordinate diagnostic work-ups, treatment initiation, and follow-up across outpatient oncology services.
- Collaborate with healthcare professionals to ensure seamless care delivery.
- Assist with prior authorizations, referrals, and test scheduling to prevent delays.
- Identify and address barriers to treatment adherence, including psychosocial and logistical challenges.
- Track patient progress, document interventions, and perform ongoing assessments and triage.
- Support care management, promote adherence, and facilitate interdisciplinary communication.
- Participate in quality improvement and workflow optimization initiatives.
- Engage in ongoing education and certification to maintain oncology competencies.
- Licensed RN with oncology experience, preferably 2-3 years.
- Certification in Oncology Nursing or willingness to obtain within specified time.
- Knowledge of outpatient oncology treatment pathways, symptom management, and triage principles.
- Strong communication, patient education, and coordination skills.
- Proficiency with electronic health records and outpatient workflows.
- Ability to work effectively in a fast-paced clinical environment with attention to detail.
