Home And Community Based Services Licensed Practical Nurse
Role & Responsibilities
Provide direct nursing care to acutely or chronically ill patients in their homes, following physician orders.
Coordinate patient care activities, document clinical notes thoroughly, and ensure high-quality service within scope of practice.
Participate in care coordination, report patient needs and changes, and communicate effectively with patients, families, and healthcare team members.
Work under supervision of a Registered Nurse or supervisor to deliver safe, compliant care.

- Strong communication, organizational, and documentation skills.
- Home care experience is advantageous.
- Flexible hours to meet client needs.
- Ability to serve clients regardless of age, race, creed, or disability.
Salary : $24 - $36
