What You’ll Do
Discharge Planning and Resident Preparation
Work one-on-one with residents during their stay in residential treatment to develop thoughtful, realistic discharge plans. Your focus is on preparation, coordination, and readiness for transition at the point of discharge.
Planning may include assistance with:
Access to benefits and entitlement programs
Transitional and long-term housing options
Aftercare and continuing care services
Transportation planning
Obtaining personal identifying documents such as state IDs and Social Security cards
Community Coordination
Maintain and strengthen relationships with community providers and external agencies to support continuity of care at discharge. Develop reliable referral pathways aligned with the organization’s mission and regulatory requirements.
Problem-Solving and Practical Support
Provide ongoing, solution-oriented assistance to residents while they remain in residential care, addressing barriers that could impact discharge readiness.
Documentation and Compliance
Document client progress and discharge planning activities in accordance with established policies and procedures
Obtain and manage releases of information
Ensure full compliance with CFR 42 and confidentiality laws when coordinating with families, guardians, and external providers
Clinical Collaboration and Education
Participate in daily clinical staffing meetings to evaluate resident progress and discharge readiness
Facilitate educational group sessions as assigned
