What are the responsibilities and job description for the RN - Weekend On Call position at PROVIDENCE HOSPICE LLC?
We are hiring a PRN Registered Nurse to join our growing team providing hospice services to patients in their homes, assisted living and long-term care facilities who are enrolled in our hospice program.
***This is an on-call weekend position ***
Responsibilities:
- Looking for someone with a Hospice heart and passion for providing end of life care
- Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG
- Responsible for overseeing patient plan of care with the collaborating physician
- Will conduct start of care visits and collaborate with Care Navigators
- Provide education to patients, families, and facility staff
Qualifications:
- Must have a RN license unincumbered in the state you are applying
- Reliable transportation and valid driver's license, clean MVR
- Experience in Hospice is preferred
