Providence Care is hiring s PRN Registered Nurse to join our growing team by providing Case Management services
to patients in their homes, assisted living and long-term care facilities who are enrolled in our hospice program.

Duties and Responsibilities
• Looking for someone with a Hospice heart and passion for providing end of life care
• Provide Case Management to patients and families, this includes symptom management, medication
management, and collaboration with members of the IDG
• Responsible for overseeing patient plan of care with the collaborating physician
• Provide supervisory visits and oversight for LPN and CNAs
• Will conduct start of care visits and collaborate with Care Navigators
• Provide education to patients, families, and facility staff
• Provide oversight to Home Health Aides and LPNs
- Must possess, as a minimum, a Nursing Degree from an accredited college or university and be licensed as a Registered Nurse in the State of application or compact licensure.
- You must have, as a minimum, two (2) years of experience as a nurse in a hospital, long-term care facility, or other related health care facility.
- Hospice experience preferred.
