Hospice Registered Nurse (RN) – Weekend On-Call

Providence Care is a trusted, community-based hospice provider serving patients and families across Georgia and South Carolina. We are seeking a compassionate and experienced Hospice Registered Nurse (RN) – Weekend On-Call to deliver high-quality, patient-centered care during critical after-hours and weekend needs.
This is an ideal role for an RN who values autonomy, flexibility, and making a real difference in end-of-life care.
What You’ll Do
- Provide on-call hospice nursing care during weekends and after-hours
- Respond to urgent patient needs including pain and symptom management
- Complete admissions, PRN visits, and follow-ups as needed
- Deliver skilled nursing care including wound care, medication management, and specialized treatments
- Coordinate care with physicians, interdisciplinary team members, and facility staff
- Educate patients, families, and caregivers on care plans and disease progression
- Provide compassionate end-of-life support to patients and families
- Maintain timely and accurate clinical documentation
- Participate in interdisciplinary team collaboration and care planning
Schedule
- Weekend On-Call coverage
- Availability for evenings, holidays, and emergency call-back as needed
Work Environment
- Patient homes, assisted living, and long-term care facilities
- Travel throughout assigned service areas in Georgia and/or South Carolina
- Fast-paced, autonomous field-based role

Qualifications:- Active, unencumbered Registered Nurse (RN) license in GA (or Compact License required)
- Minimum 2 years of RN experience (hospital, long-term care, or related setting)
- Hospice or home health experience preferred
- Strong clinical judgment and ability to work independently in the field
- Excellent communication and interpersonal skills
- Comfort supporting patients and families through end-of-life care
