Position Summary
The Hospice Registered Nurse – Weeknight On-Call provides skilled nursing care, clinical coordination, and on-call support to hospice patients during evening and overnight hours. This role ensures timely response to patient needs, symptom management, admissions support, and compassionate care during critical moments.
Key Responsibilities

Provide skilled hospice nursing care in accordance with physician orders and individualized plans of care

Respond to after-hours patient needs, symptom crises, and urgent clinical concerns

Perform admissions, follow-up visits, and transition-of-care support as needed

Coordinate care with physicians, interdisciplinary team members, facility staff, patients, and families

Provide education and guidance to patients, families, caregivers, and facility staff

Offer emotional support to patients and families, including presence at time of death when needed

Ensure accurate, timely documentation to support quality care and regulatory compliance

Participate in interdisciplinary team communication and quality improvement efforts

Supervise patient care aides and support volunteers as assigned

Maintain patient rights, confidentiality, dignity, and respect at all times

Active, unencumbered Registered Nurse (RN) license in the State of South Carolina

Graduate of an accredited nursing program

Minimum of two (2) years of nursing experience (hospice experience preferred)

Strong clinical judgment and ability to work independently during on-call hours

Excellent communication, organization, and documentation skills

Compassionate, calm, and professional approach in emotionally sensitive situations

Ability to travel to patient homes and facilities as required
