About the Role
Providence Care is seeking a compassionate and dedicated Hospice Chaplain & Bereavement Coordinator to provide spiritual support and bereavement care to our patients, families, and caregivers. This role ensures that spiritual and bereavement services align with federal, state, and local regulations while serving as an integral part of the interdisciplinary hospice care team.
Key Responsibilities
Chaplain Functions
- Assess patients' and families' spiritual needs within five days of admission
- Develop and update a spiritual care plan in alignment with the hospice care plan
- Provide pastoral care, counseling, and religious sacraments as appropriate
- Collaborate with the interdisciplinary team to ensure holistic patient care
- Build relationships with community clergy to facilitate additional chaplaincy support
- Offer individual, family, or group counseling as needed
- Participate in community education initiatives about hospice care
Bereavement Coordinator Function
- Lead and manage the bereavement services program
- Develop and oversee grief support programs for patients' loved ones
- Conduct bereavement assessments and provide counseling or support referrals
- Organize memorial events and grief support groups
- Maintain accurate bereavement documentation and ensure compliance with hospice policies
- Train and oversee volunteers assisting in bereavement services
Additional Responsibilities
- Support hospice staff through counseling and structured meetings
- Participate in hospice committees and interdisciplinary meetings
- Assist in orientation and ongoing education programs for staff
Education
Master's Degree in Divinity or a related theological field from an accredited seminary or school of theology preferred. At least one unit of Clinical Pastoral Education (CPE) is required.
Experience
Minimum of two years of pastoral leadership and/or chaplaincy experience. Experience working with terminally ill patients and their families is preferred.

- Strong communication and interpersonal skills
- Ability to provide compassionate support to patients and families
- Familiarity with hospice care philosophy, regulations, and best practices
- Capable of handling sensitive and emotionally charged situations with professionalism
- Proficiency in documentation and record-keeping
