We are currently seeking a skilled and compassionate Registered Nurse (RN) to join our home health team.
Position Summary
The Home Health Visiting RN is responsible for providing skilled nursing services to patients in their homes in accordance with the physician’s plan of care, Medicare Conditions of Participation, and agency policies. This role includes patient assessment, care coordination, education, and clinical documentation to ensure safe, effective, and compliant home health services.
Key ResponsibilitiesPatient Care & Clinical Services
- Perform Start of Care (SOC), Resumption of Care (ROC), recertification, and discharge assessments as assigned.
- Conduct comprehensive and ongoing patient assessments in the home setting.
- Develop, implement, and revise individualized Plans of Care in collaboration with physicians and the interdisciplinary team.
- Provide skilled nursing care including, but not limited to:
- Wound and ostomy care
- Medication management and administration
- IV therapy and infusion services (as credentialed)
- Chronic disease management
- Patient safety and fall risk interventions
- Monitor patient progress, identify changes in condition, and report findings promptly to the physician.
- Provide patient and caregiver education related to disease processes, medications, safety, and self-management.
Documentation & Regulatory Compliance
- Complete accurate, timely, and compliant documentation in the agency’s Electronic Medical Record (EMR) system.
- Ensure documentation meets Medicare, Medicaid, and payer requirements, including OASIS assessments when applicable.
- Follow all Provide Home Health policies, procedures, and quality standards.
- Participate in quality improvement activities as required.
Care Coordination & Communication
- Collaborate with physicians, LVNs, therapists, medical social workers, home health aides, and other care team members.
- Communicate effectively with patients, families, caregivers, and referral sources.
- Participate in interdisciplinary case conferences and team meetings as scheduled.
- Support continuity of care through effective coordination and follow-up.
QualificationsRequired
- Active California Registered Nurse (RN) license in good standing.
- Current CPR/BLS certification.
- Reliable transportation, valid driver’s license, and auto insurance.
- Ability to work independently in a home health environment.
Preferred
- Home health experience (SOC, ROC, OASIS).
- IV therapy experience or certification.
- Strong familiarity with Medicare home health regulations.
- EMR experience.
