Licensed Vocational Nurse

PROVIDE HOME HEALTH is a compassionate and patient-focused home health agency dedicated to delivering high-quality medical care in the comfort of our patients’ homes. We are seeking a Licensed Vocational Nurse (LVN) to join our growing team and provide exceptional care to our patients.

Job Summary

The Home Health Visiting LVN is responsible for providing skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). This role requires excellent clinical judgment, strong communication skills, and the ability to work independently while following the established plan of care.

Key Responsibilities Patient Care & Clinical Duties:

• Provide hands-on nursing care to home health patients based on individualized care plans.
• Perform assessments, monitor vital signs, and administer medications as prescribed.
• Assist with wound care, dressing changes, injections, catheter care, and other skilled nursing tasks.
• Educate patients and caregivers on disease management, medication adherence, and self-care techniques.
• Coordinate with RNs, therapists, and physicians to ensure continuity of care.

Documentation & Compliance:

• Maintain accurate and timely electronic medical records (EMR) in accordance with agency policies and home health regulations.
• Document all visits, patient conditions, and any changes in the plan of care.
• Ensure compliance with Medicare, Medicaid, and private insurance regulations.
• Report any changes in patient status to the supervising RN or physician promptly.

Communication & Coordination:

• Build strong relationships with patients and their families to provide compassionate and patient-centered care.
• Communicate effectively with the home health team to coordinate patient care and follow-up.
• Participate in interdisciplinary team meetings as needed.

Qualifications & Requirements

• Licensure: Active LVN license in California.
• Experience: Prior home health experience preferred, but not required. Training provided for new graduates with strong clinical skills.
• Certifications: Current CPR/BLS certification required. IV certification is a plus.
• Skills:
• Strong assessment, clinical, and documentation skills.
• Ability to work independently while following a care plan.
• Excellent communication and interpersonal skills.
• Familiarity with EMR systems and home health documentation.
