Position Summary

Key Responsibilities
- Evaluate, diagnose, and treat patients in accordance with established clinical standards of care
- Provide comprehensive primary care services with a focus on senior populations
- Deliver patient-centered care with an emphasis on quality outcomes
- Coordinate patient services including:
Hospital and Skilled Nursing Facility (SNF) transitions
Durable medical equipment
Home health care services
- Actively participate in daily team huddles as a key clinical expert
- Collaborate with physicians, advanced practice providers, pharmacists, behavioral health specialists, and care teams
- Support a positive and cooperative work environment alongside the Lead Physician and Center Administrator
- Complete accurate and timely medical record documentation daily
- Partner with quality-based coders to ensure optimized documentation|
- Follow all clinical policies, protocols, and regulatory requirements
- MD or DO from an accredited medical school
- Current and unrestricted medical license (or willingness to obtain licensure in Mississippi and other assigned regions)
- Board Certified or Board Eligible (ABMS or AOA) in:
- Family Medicine
- Internal Medicine
- Geriatric Medicine (preferred)
- Active and unrestricted DEA license
- Medicare and Medicaid Provider Numbers
- Strong interpersonal and communication skills
- Guaranteed base salary plus performance-based bonus
