Key Responsibilities
- Conduct comprehensive health assessments, histories, and physical examinations
- Diagnose and manage acute and chronic adult conditions
- Manage chronic illnesses such as diabetes, hypertension, asthma, and cardiovascular disease
- Coordinate care with specialists and behavioral health providers
- Support compliance with FQHC and HRSA standards
- Assist in tracking productivity and quality metrics
- Maintain timely and compliant EHR documentation
- Participate in on-call rotation as required
- Master's degree or higher from an accredited NP program (Adult, Adult-Gerontology, or FNP with adult focus)
- Active New York State Nurse Practitioner license
- National NP certification (AANP or ANCC)
- DEA registration and NPI (or ability to obtain)
- Current CPR/BLS certification
- Minimum 2 years of adult primary care experience
- Strong diagnostic and clinical assessment skills
- Experience with electronic health records (HER)
- Experience in an FQHC, community health center, or safety-net setting
- Starting Salary: $150,000 annually
