Position Overview

We are seeking an experienced Clinical Pharmacist to join our hospital pharmacy team in Ottumwa, Iowa. This role is responsible for providing safe, effective, and evidence-based medication therapy while supporting both centralized and decentralized pharmacy services.

The Clinical Pharmacist works collaboratively with physicians, nurses, and other healthcare professionals to optimize patient outcomes through comprehensive pharmaceutical care, medication therapy management, and clinical interventions.

Key Responsibilities

• Provide direct patient-centered pharmaceutical care across a variety of disease states.
• Review, verify, and optimize medication therapy for hospitalized patients.
• Assess patient-specific medication needs and develop individualized treatment recommendations.
• Monitor medication effectiveness and adjust therapy based on patient response.
• Identify, prevent, and resolve medication-related problems and adverse drug events.
• Serve as a preceptor for Doctor of Pharmacy (PharmD) students
  
  

Qualifications

• Licensed as a Registered Pharmacist in the State of Iowa
• Graduate of an accredited College of Pharmacy
• Minimum of 2-4 years of clinical pharmacy experience in a hospital setting
• Strong clinical knowledge and patient care skills
