This position is Monday - Friday, 8:30 Am to 4:30 PM snd located at the Stephen Klein Wellness Center, 2144 Cecil B. Moore Philadelphia, PA.
What You'll Do
As a Patient Support Referral Coordinator, you will play a key role in ensuring patients receive timely, coordinated care.
- Manage referrals from provider order through specialist follow-up, ensuring continuity of care
- Assist uninsured or underinsured patients with insurance applications and financial documentation
- Track and follow up on hospital admissions and referrals to support patient outcomes
- Answer incoming calls, schedule appointments, and support front desk operations as needed
- Provide clerical and administrative support to ensure smooth clinic workflow
- Collaborate with care teams to meet Patient-Centered Medical Home standards

What You Bring
- High school diploma or GED with at least 2 years of relevant experience
- Strong organizational and computer skills with the ability to manage multiple priorities
- Excellent communication skills and professional phone presence
- Experience with EHR, practice management, or dental record systems (NextGen or similar preferred)
- Ability to work both independently and as part of a collaborative team
- Experience working with underserved populations is highly valued
- Bilingual Spanish/English is a plus
