Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team in Whitefield, New Hampshire, for evening shifts. The ideal candidate will provide high-quality patient care across various settings, including long-term care, assisted living, and acute care environments. This role requires a compassionate approach, strong clinical skills, and the ability to work effectively within multidisciplinary teams. The LPN will be responsible for patient assessments, medication administration, and supporting patient comfort and safety while adhering to HIPAA regulations and documentation standards.
Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and health history documentation using EMR/EHR systems such as Epic, Cerner, or Athenahealth.
- Administer medications and injections accurately following physician orders and safety protocols.
- Manage IV infusions, catheterizations, and other specialized procedures with aseptic technique.
- Assist with diagnostic evaluations such as blood draws (venipuncture), specimen collection/processing, and radiology procedures like sonography or medical imaging when applicable.
- Provide specialized care for patients with complex needs including dementia care, Alzheimer's care, memory care, hospice care, and pain management.
- Support patients with developmental disabilities and those requiring behavioral health interventions or post-acute care.
- Document all patient interactions thoroughly in compliance with medical records standards and HIPAA regulations.
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans including discharge planning and case management.
- Maintain infection control standards and ensure sterile processing of equipment.
- Participate in patient education on health coaching, nutrition, medication management, and home & community care instructions.
Experience
- Proven experience working in diverse healthcare settings such as hospital medicine, ICU, emergency room, trauma centers (Level I & II), pediatric units (PICU), neonatal ventilator units, or outpatient clinics.
- Strong background in critical care experience including acute pain management, infectious disease care, ventilator management, airway management, spinal tap procedures, and cardiac catheterization is preferred.
- Familiarity with ICD-9/10 coding systems for medical documentation and CPT coding for procedures is advantageous.
- Experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks is required.
- Knowledge of physiology, anatomy, pharmacology related to medication administration including tube feeding and dermal fillers is beneficial.
- Prior experience working with vulnerable populations such as children (including neonatal), elderly patients requiring geriatrics or memory care services is highly desirable.
- Certifications in Basic Life Support (BLS) are mandatory; additional certifications like Phlebotomy or Sterile Processing are a plus.
