Position Overview:
We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to serve as a Case Manager in the Greater Lake Charles area. This role focuses on managing a designated caseload of hospice patients, ensuring delivery of patient-centered care that supports comfort, dignity, and quality of life. The ideal candidate brings hospice experience, strong care coordination skills, and a positive, flexible approach to patient care.
Schedule & Work Details:
Location: Greater Lake Charles area
Schedule: Monday through Friday, 8:00 AM to 4:30 PM
Weekly Hour Guarantee: 40 hours
On Call: None
Weekend Requirement: None
Mileage Reimbursement: Yes, reimbursed between patient visits from first to last visit
Pay Rate: $51 to $53 per hour
Key Responsibilities:
- Manage a caseload of hospice patients, serving as the primary clinical point of contact
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
- Perform comprehensive assessments and ongoing evaluations of patient condition and needs
- Coordinate care services including nursing, aides, social work, and other support services
- Provide symptom management, pain control, and end-of-life care
- Educate patients and families on disease progression, medications, and care expectations
- Maintain accurate and timely documentation in Homecare Homebase (HCHB)
- Ensure compliance with hospice regulations, standards, and best practices
Required Qualifications:
- Active Registered Nurse (RN) license
- Hospice experience required
- Strong clinical assessment and case management skills
- Valid driver’s license and reliable transportation
Preferred Qualifications:
- Experience with Homecare Homebase (HCHB)
- Strong patient-centered mindset with a positive, flexible attitude
- Excellent communication and interpersonal skills
- Ability to manage time effectively and work independently
