The Home Health Registered Nurse provides skilled clinical care to adult patients (18 ) in the home health setting. This role focuses on delivering high-quality, patient-centered nursing care in accordance with physician orders and individualized plans of care. The RN works independently in the field while collaborating with the interdisciplinary care team to support safe transitions, improved outcomes, and continuity of care.
Schedule & Work Details- Schedule: Monday through Friday
- Hours: 8:00 am to 4:30 pm
- Weekly Hour Guarantee: 40 hours
- Coverage Area: Lake Charles, LA and surrounding areas within a 50-mile radius
- On Call Requirement: One weekend per month and one weekday per week
- Mileage Reimbursement: Yes, reimbursed between patient visits from first to last visit
- Pay Rate: $51 to $53 per hour
- Provide skilled nursing care to adult patients in their homes in accordance with physician orders and established plans of care
- Perform comprehensive patient assessments during start of care, follow-up, recertification, and discharge visits
- Deliver direct clinical care including medication administration, monitoring patient responses, and managing ongoing health needs
- Observe, assess, and document changes in patient condition and communicate findings promptly to physicians and the care team
- Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management
- Ensure all clinical documentation is accurate, complete, and submitted in a timely manner in compliance with regulatory and agency standards
- Coordinate care with physicians, therapists, case managers, and other interdisciplinary team members
- Maintain compliance with quality, safety, and productivity expectations
- Participate in on-call coverage as scheduled
- Active, unencumbered Registered Nurse license
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
- Current Basic Life Support (BLS) certification
- Ability to work independently in patient homes and manage a field-based caseload
- Valid driver’s license and reliable transportation
- Proficiency with Homecare Homebase (HCHB)
- Prior experience working with LHC
- Strong clinical assessment and critical thinking skills
- Effective communication and time management abilities
