Position Summary:
The Community Liaison serves as the primary relationship builder between PM Home Health / PM Residence and
the surrounding healthcare and referral community. This role is responsible for developing strategic partnerships,
increasing referrals, supporting census growth, and ensuring seamless communication between referral sources,
clients, families, and internal teams.
The Community Liaison represents the organization in the community, strengthens brand awareness, and ensures
that potential clients and healthcare partners clearly understand our services, philosophy of care, and commitment
to excellence.
Primary Job Responsibilities:
· Determine primary decision-makers within referral sources and proactively build and maintain strong
professional relationships with
o Physicians
o Case managers
o Discharge planners
o Social workers
o Skilled nursing facilities
o Hospitals
o Community organizations
· Make regular, planned outreach calls and in-person visits to:
o Secure new referrals
o Maintain existing referral relationships
o Build census
o Meet established growth goals
· Conduct initial meetings with clients and families to:
o Provide an overview of services and philosophy
o Explain care options
o Obtain required consents for care
· Implement, manage, and document consistent sales and outreach activities across multiple referral
sources.
· Maintain accurate records of outreach efforts, referral activity, and client interactions to inform strategic
adjustments
· Seek to understand referral partner and client needs in order to provide customized solutions and
maintain long-term partnerships.
· Expand community awareness and utilization of PM Home Health / PM Residence services.
· Serve as a liaison between referral sources and clients/families to ensure smooth communication and
transitions of care.
· Represent PM Home Health / PM Residence at:
o Community events
o Health fairs
o Networking functions
o Industry meetings
· Collaborate with internal teams to ensure seamless service delivery while meeting growth and outreach goals.
· Gather feedback from clients and community partners to continuously improve outreach strategies and service delivery.
Qualifications Required:
· Bachelor’s degree in healthcare administration, marketing, business, social work, or related field preferred.
· 2–5 years of experience in healthcare marketing, admissions, community relations, or business development.
· Demonstrated ability to build and maintain professional relationships.
· Strong communication and presentation skills. · Knowledge of home health, personal care, memory care, or long-term care services preferred.
· Proficiency in CRM systems, outreach tracking, and basic reporting tools
Preferred Qualifications:
· Established network within the local healthcare community.
· Experience working with hospitals, SNFs, assisted living facilities, or case management teams.
· Sales or census growth experience in healthcare environment.
· Clear understanding of healthcare compliance and HIPAA requirements.
Physical & Working Conditions:
· Community-facing role requiring regular local travel.
· Frequent in-person meetings and networking events. · Flexible schedule may be required to accommodate community events.
· Primarily field-based with some administrative responsibilities.
Salary : $52,000 - $60,000
