Conduct comprehensive assessments of residents' health status, including physical, mental, and emotional well-being
Collaborate with interdisciplinary teams, including physicians, nurses, therapists, and social workers, to develop personalized care plans for residents
Ensure accuracy and compliance with regulatory requirements in resident assessments and care planning documentation
Coordinate and facilitate care planning meetings to discuss residents' progress, goals, and care plan adjustments
Provide ongoing education and support to staff members regarding assessment processes, documentation requirements, and best practices in resident care
Registered Nurse Assessment Coordinator (RNAC) Job Requirements:
Graduate of an accredited School of Nursing (RN)
Current/active license for Registered Nurse (RN)
Clinical Experience in a nursing facility
Demonstrated clinical assessment skills
Knowledge of Medicare and Medicaid regulations and coverage guidelines as related to skilled nursing
