LNAC Experience Preferred But Will Train.
Long term care experience preferred
The LNAC coordinates the MDS process in accordance with regulatory and financial requirements following the resident assessment instrument (RAI), and works as part of the interdisciplinary team to ensure the highest quality care is being provided to the residents
Duties include
Coordination and completion the MDS, CAA and care planning process
Diagnosis coding
Ensuring resident care plans are current and reflect the information needed to provide quality care to the resident.
Participation in resident care conferences.
Other duties as assigned
Knowledge of MDS 3.0, CAAS, Rugs, PDPM and CMI preferred.
Must have current LPN license
Must have some acute and/or long term care experience.
Need strong leadership, clinical and interpersonal skills
Hours
Monday through Friday daylight.
Must be willing to work an occasional weekend day or holiday on the nursing unit, to assist with RN management coverage if needed.
