Restorative Nurse (LPN) Responsibilities:
- Implement individualized care plans that target specific functional and restorative goals.
- Assist residents with Activities of Daily Living (ADLs) including walking, dressing, grooming, eating, transferring, communication, and use of assistive devices
- Supervise and demonstrate passive and active range of motion exercises, splint or brace application, mobility training, and bladder/bowel continence training
- Encourage residents to perform self-care to maximize autonomy and reduce dependency on staff
- Communicate with residents, families, and the healthcare team about care plans, progress, and any changes in condition
- Advocate for resident rights, ensuring participation in treatment decisions and maintaining privacy, dignity, and self-determination
- Promote resident safety while performing exercises and tasks, preventing falls, pressure injuries, and other complications
- Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania
- Prior experience in long-term care, rehabilitation, or a related field preferred
- Ability to adapt to changing priorities and situations in a fast-paced environment
- Excellent communication, interpersonal, and problem-solving skills
