Registered Nurse Assessment Coordinator (RNAC) Responsibilities:
- Conduct thorough assessments of patients' medical, social, and emotional needs, and develop individualized care plans to address their unique requirements.
- Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care delivery and effective communication with patients and families.
- Develop and implement effective interventions to promote patient independence, safety, and well-being.
- Collaborate with the care team to identify and address potential risks and concerns, and implement strategies to prevent hospitalization or rehospitalization.
- Maintain accurate and up-to-date records of patient assessments, care plans, and progress notes.
- Current Pennsylvania RN license
- Prior years of experience as a Registered Nurse, with a focus on geriatric care and assessment
- Strong assessment and communication skills, with the ability to work effectively with patients, families, and healthcare teams
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong organizational and time management skills, with excellent attention to detail and accuracy
