Registered Nurse (RN) for Home Health Care Responsibilities:
- Provide high-quality, patient-centered care to patients in their homes, focusing on symptom management, education, and support
- Conduct thorough assessments, develop individualized care plans, and implement interventions to promote optimal health and wellness
- Collaborate with patients, families, and interdisciplinary team members to ensure comprehensive care
- Maintain accurate and timely documentation of patient care and treatment outcomes
- Participate in quality improvement initiatives and contribute to the development of best practices in home health care
- Stay current with industry trends, research, and regulatory requirements to ensure compliance and excellence in care
- Current licensure as a Registered Nurse (RN) in the state of Pennsylvania
- Minimum one year experience in home health care or a related field
- Experience with completing the OASIS assessment strongly preferred
- Strong clinical skills and knowledge of adult and geriatric care
- Excellent communication, interpersonal, and organizational skills
- Ability to work independently and as part of a team
