Utilize Net Health and PointClickCare (PCC)
Position Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services in a SNF/LTC setting
- Collaborate with interdisciplinary team members to maximize resident outcomes
- Complete timely and accurate documentation using Net Health
- Educate residents, families, and caregivers on treatment plans and progress
- Maintain compliance with all regulatory and facility requirements
- Graduate of an accredited Physical Therapy program
- Current PT license in the State of Pennsylvania (or eligible)
- Strong communication and interpersonal skills
- Passion for providing exceptional patient-centered care
- Experience in SNF/LTC preferred, but not required
- New graduates are welcome and encouraged to apply
