- Provide skilled occupational therapy services in a SNF/LTC setting
- Help residents improve independence with activities of daily living and functional mobility
- Collaborate with nursing, therapy, and interdisciplinary team members to achieve optimal outcomes
- Complete timely and accurate documentation using Net Health
- Educate residents, families, and caregivers regarding treatment goals and progress
- Maintain compliance with facility policies and regulatory requirements
- Graduate of an accredited Occupational Therapy program
- Current Pennsylvania OT License or eligibility for licensure
- Strong communication and interpersonal skills
- Ability to work effectively in a team-oriented environment
- SNF/LTC experience preferred but not required
- Passion for resident-centered care and clinical excellence
