Admissions Coordinator Responsibilities:
- Coordinate and facilitate the admission process for new patients, ensuring efficient and effective communication with patients, families, and healthcare providers
- Conduct admissions assessments to gather necessary information and complete required paperwork
- Build relationships with healthcare professionals, insurance companies, and community organizations to promote our services and referring patients to our center
- Utilize electronic medical records to document and track patient information
- Collaborate with the interdisciplinary team to ensure a comprehensive understanding of patients' needs and develop individualized care plans
- Provide exceptional customer service to patients, families, and caregivers to ensure a positive experience during the admission process
- Min 2 years of experience in a SNF, in the Admissions Department
- Education in a healthcare-related field preferred
- Must have knowledge/experience with Medicare, Medicaid, Insurances and be able to complete verifications.
- Must have strong PR skills, be self-directed, able to manage a busy environment.
- Requires some weekends
