Physical Therapist Position Summary:  

Under the direct supervision of the Rehab Director the primary purpose of your job position is to provide appropriate aspects of quality patient care by providing therapy services and evaluating/assessing needs and formulating/carrying out treatment plans for patients in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility.  

Education and Licensure:   

• Graduate of an approved curriculum in Physical Therapy  
• Working knowledge of Medicare and other payor sources.  
• Is in good standing with all regulatory agencies and licensing boards.  
• Previous experience in a skilled nursing facility preferred.  
• CPR Certification, as required for license renewal.
