We are seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in their homes or senior living communities using our Geriatric House Calls™ model.

- Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and overall wellness.
- Perform therapy sessions, educate patients and caregivers, and document care accurately within our EMR system.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.
- Utilize telehealth and virtual care options to enhance access and continuity of care.
- Valid PT license in applicable state(s) and degree from an accredited program.
- Proficiency in basic computer and EMR use, current CPR certification.
- Ability to meet physical demands, including lifting, repositioning patients, and driving to patient locations.
- Autonomous role with flexible scheduling and meaningful patient relationships.
