Physical Therapist, Home Care

Overview

We are seeking a Physical Therapist to provide personalized, independent rehabilitative care in patients' homes and communities. You will assess patient needs, develop treatment plans, and adjust care based on evolving conditions, working as part of an interdisciplinary team. Responsibilities include conducting comprehensive assessments, delivering therapy, and monitoring progress to ensure optimal recovery.

Qualifications & Skills

• Minimum one year of physical therapy experience or successful completion of probation as a Rehab Intern
• Licensure to practice in New York State
• Valid driver’s license required outside Manhattan
