IMPACCT Brooklyn is seeking a full-time Social Services Case Manager to oversee a diverse caseload of clients. The Case Manager will provide personalized support to residents using a strength-based approach that empowers individuals and families to achieve stability and well-being. This role focuses on case counseling, connecting residents to essential resources, and promoting community engagement through programming for seniors and youth. Key Responsibilities Include: 
Case Counseling & Support

  • Conduct assessments to identify resident strengths, needs, and goals.
  • Provide short-term counseling and guidance to support residents in achieving personal and housing stability.

 

Resource Navigation & Referrals

  • Make referrals for mental health and substance abuse services.
  • Assist residents in accessing benefits such as SNAP, SSI, and rental assistance.
  •  Connect residents to food pantries and other emergency food resources.

 

Community Programming

  • Collaborate on developing and implementing programs for seniors and youth to promote engagement and well-being.
  • Support residents in participating in workshops, social activities, and educational opportunities.

 

Linkage Development

  • Establish and maintain partnerships with local service providers, nonprofits, and community organizations.
  • Identify new resources and opportunities to enhance resident support services.

 

Documentation & Reporting

  • Maintain accurate case notes, service plans, and referral records in compliance with organizational standards.
  • Prepare reports on resident progress, program participation, and community needs.

Qualifications:

Skills & Competencies 

  • An associate or bachelor’s degree in social work, human services, psychology, or a related field is preferred.
  • A minimum of 1-2 years of experience in social services, case management, or counseling is required.
  • Knowledge of mental health and substance abuse resources, as well as public benefits programs, is essential.
  • Experience working with diverse populations and a solid understanding of community resources is highly beneficial.
  • Certification in case management (e.g., CCM, ACM) is a plus.
  • A strong understanding of social service programs, community resources, and the needs of various populations is important.
  • Proficiency in using case management software, databases, and standard office applications (e.g., MS Office) is required.
  • Strong verbal and written communication skills are necessary for effective interaction with clients, families, and professionals.
  • Excellent organizational skills and the ability to maintain accurate records are essential.
  • The ability to respond to emergencies and manage crises calmly and effectively is crucial.
  • Awareness and sensitivity to cultural differences, along with the ability to work effectively with diverse populations, is important.
  • A commitment to ethical practice and maintaining confidentiality in all client interactions is required.
  • The ability to collaborate as part of a team while fostering positive working relationships, as well as the capacity to work independently, is essential.
  • Fluency in the English language is required; bilingual proficiency in Spanish is a plus.

None

 

Employment Status
Non-Exempt, Full-time. The days and hours of work are primarily Monday through Friday, 9:00 a.m. to 5 p.m. Travel may be expected. Occasional evening or weekend work and local travel may be required for events or funder meetings.

Physical Demands and Work Environment 
This job operates in a professional office environment.  This role involves a mix of office tasks and direct participant engagement. The physical demands described here are those an employee must meet to perform the essential functions of this position successfully.  Reasonable accommodation may be provided to enable individuals with disabilities to perform the functions.  The role requires the ability to stand or sit for extended periods at a desk, work on a computer, and occasionally lift materials weighing up to 25 pounds.  Mobility is needed to navigate the office and assist participants during programs.  Typically based in a standard office setting, the environment has a noise level that is usually moderate to high, depending on the time of day and client population.  Regular interaction with clients and staff fosters a supportive atmosphere, and the noise level is usually moderate to high, depending on the time of day.
