**POSITION DESCRIPTION**

This position description is a temporary management guide tool and is

subject to change.

**POSITION GOAL**

The Assertive Community Treatment (ACT) team provides non traditional

community based mental health treatment to promote the recovery and

community integration of adults with serious and persistent mental

illnesses and co occurring substance use and/or serious cooccurring

physical health conditions. The overall goal of the ACT team is to

reduce use of ERs, inpatient hospital stays, and other harmful

behaviors. The ACT Peer Support Specialist is an integral part of a

multidisciplinary team of mental health clinicians/providers. This

position will provide patient centered services with a rehabilitation

and recovery focus that are designed to promote skills for coping with

and managing psychiatric symptoms, education regarding daily living

skills and overall wellness, and other matters related to assisting

participants to live more independent lives. The ACT peer support

specialist will be expected to manage his/her own schedule and will be

expected to work in a team environment. The vast majority of services

will take place in participants homes and/or in the community. Will be

required to be on call for a 7 day rotation every 6 to 8 weeks.

  • CPA 1: PEER SUPPORT COMPONENTS \***
  • Help the service participant to develop a network for information

and support from others who have been through similar experiences.

  • Articulate what has been helpful and what was not helpful in

his/her own recovery.

  • Identify beliefs and values a patient holds that work against

his/her recovery.

  • Discern when and how much of their recovery story to share and

with whom.

  • Discuss his/her own tools for taking care of himself/herself.
  • Assist the service participants with regaining the ability to make

independent choices and to take a proactive role in treatment,

including discussing questions or concerns about medications,

diagnoses, or treatment approaches with their treating clinician.

  • Create and facilitate a variety of group activities that support

and strengthen recovery.

  • Complete necessary documentation required by the state.
  • Help a service participant combat negative self talk, overcome

fears, and solve problems.

  • Help a service participant articulate, set, and accomplish

his/her goals.

  • Teach other service participant to advocate for the services

that they want.

  • Help a service participant create a Person Centered Plan.
  • Assist the service participant with identifying and effective

responding to or avoiding identified precursors or triggers that

result in functional impairments.

  • Ask open ended questions that relate a person to his/her inner

wisdom.

  • Demonstrate an ability to participate in healing communication.
  • Interact sensitively and effectively with people of other

cultures.

  • Teach other patients to create their own Wellness Recovery

Action Plan.

  • CPA 2: ADMINISTRATIVE RESPONSIBILITIES \***
  • Accurately complete E time on a daily basis.
  • Maintain confidentiality in all aspects of position.
  • Communicate with community partners only after obtaining proper

authorization.

  • Complete paperwork tasks within required time frames.
  • Attend nd regularly participate in ACT team meetings.
  • Attend and regularly participate in clinical and administrative

supervision.

  • Maintain responsibility for organizing a schedule and establishing

priorities, according to existing patient and agency needs.

  • Maintain adequate and timely documentation in the clinical record of

treatment interventions and other activities related to service

participant care, as defined by Prairie View standards.

  • Schedules are completed within 24 hours of the service provided.
  • Progress notes are completed within 72 hours.
  • CPA 3: PRODUCTIVITY EXPECTATIONS \***
  • Maintain 24 to 27 encounters weekly or 1,152 to 1,296 encounters

annually.

  • Ensure that contacts with service participants are goal directed,

outcome focused, and meaningful.

  • CPA 4: AGE BASED COMPETENCY \***

Acknowledges the rights and responsibilities of adult service

participants.
