Job Summary
We are seeking a motivated LPN or RN to join our team to work as our Admissions/Infection Control Nurse. This is an 8-5, M-F shift with a requirement to be the On-Call nurse every 4 weeks. The week that you are on call, carries a bonus.
Responsibilities
- Facilitate smooth transfer from the hospital to our facility
- Provide an initial baseline for the skilled service documentation for rehab residents so nurses can document on key issues for the resident
- Provide nursing chart reviews to identify residents who need documentation to support quality care
- Coordinate with the Wound Care on treatment plans and documenting progress
- Track and trend antibiotic usage by microbe and evaluate for any spread
- Conduct regular patient monitoring and observation to identify potential infection risks
- Manage infection prevention programs and ensure compliance
- Perform staff competencies
- Analyze data related to infection rates and trends, providing actionable insights to improve patient care
Requires an Active and Unrestricted LPN or RN License

Pay: $28.00 - $30.00 per hour
