RN Case Manager Job Details
- Position Type: Locum tenens (travel contract)
- Healthcare Setting: Inpatient Rehabilitation Facility (IRF)
- Location: Coralville, IA 52241
- Shift: 8:00 AM – 4:30 PM (8-hour days)
- Hours per Week: 40
- Assignment Duration: 13 weeks (Start: August 11, 2025 – End: November 10, 2025)
- Unit: IRF Division
- Number of Beds: 40
- Estimated Weekly Salary: $2120 - $2180
- Active Registered Nurse (RN) license – Iowa or compact state
- Minimum 1 year of case management experience required; 2 years in inpatient rehabilitation or acute care preferred
- Experience in rehabilitation hospital or acute care settings preferred
- Strong oral and written communication skills in English; bilingual skills a plus
- Proficiency with healthcare software and basic computer skills (Excel, Word, Outlook, PowerPoint)
- Excellent organizational, time management, and analytical abilities
- Ability to work independently and collaboratively within a healthcare team
- Commitment to patient confidentiality and compliance with HIPAA regulations
- Coordinate and manage patient care throughout the inpatient rehabilitation process
- Develop and implement individualized care plans for patients with stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injuries
- Collaborate with interdisciplinary healthcare teams to optimize patient outcomes, function, and quality of life
- Communicate effectively with patients, families, and healthcare professionals
- Maintain accurate documentation and ensure compliance with hospital and healthcare policies
- Support patients and families through recovery, providing education and resources
